Patient Safety First with ADN

The Admission Discharge Notification notifies the patient’s General Practitioner within 24 hours of the person presenting and being discharged from hospital. 

This in turn allows GPs to recall or organise appointments ensuring the continuity of care for their patients.

The Admission Discharge Notification improves the long-term health of Mid North Coast community by delivering the right care, at the right time and at the right place.

Avoiding the PITS

This project aims to reduce the exposure of immunosuppressed patients and families to patients presenting to the Emergency Department with COVID-19 symptoms.

Through the introduction of a nurse-supported triage position, patients with suspected febrile neutropenia and other treatment-related illnesses are assessed via telephone and provided with alternative care options to avoid Emergency Department presentation.

Telephone triage enabled patients receiving treatment within the MNCCI to be managed at home or within the Mid North Coast Cancer Institute treatment centre by senior clinicians.

Patient Flow Optimising System

The orange tag Patient Flow Optimising System is a visual reminder for Operations Assistants to retrieve the appropriate bed at the end of a patient’s procedure.

The system ensures patients are placed on the correct ward bed, post-operation to minimise pain, discomfort and trauma caused by transferring from one bed to another.

This project has reduced the need for patients to move from their Day Surgery bed to a ward bed post-operation, and improved workplace safety for the Perioperative Team.

Making the Statistics Fall

The reduction of patient falls in an acute mental health setting requires a multi-faceted approach to prevent injury. 

This project implemented a range of measures including: regular staff education on falls management, a quick reference guide for falls assessment, the inclusion of falls prevention strategies included in care planning, and falls risks noted on the electronic journey Board.

Through the implementation of these strategies a 30 per cent reduction of falls was achieved.
